Job Title: Care Coordinator

Reports To: Registered Manager / Senior Management

Purpose of Role:

To ensure the smooth planning, coordination, and delivery of high-quality care services to clients, while supporting care staff and maintaining excellent communication with service users and their families.

Principal Responsibilities:

  • Coordinate daily care schedules for care staff, ensuring timely and appropriate service delivery.
  • Communicate with clients and families to assess needs and update care plans.
  • Supervise, support, and mentor care staff, promoting high standards of care.
  • Conduct spot checks and service quality audits to ensure compliance.
  • Maintain accurate records, including staff rotas, client notes, and incident reports.
  • Respond to emergency scheduling changes or staffing issues promptly.
  • Liaise with healthcare professionals, social workers, and other agencies.
  • Support the recruitment, onboarding, and training of new care staff.
  • Uphold all safeguarding, confidentiality, and data protection protocols.
  • Report concerns or incidents to the Registered Manager without delay.
